In accordance with The Saskatchewan Polytechnic Act (“the Act”) and The Local Authority Freedom of Information and Protection of Privacy Act (“LA FOIP”), Saskatchewan Polytechnic collects, uses and discloses Personal Information to perform its duties under the Act; as permitted by LA FOIP.

The personal information on this form is collected for the purpose of registering for testing services. Your information will be kept confidential and may be shared internally for administrative and academic programming related to the testing service being provided.
